Transcript Requests/Records


A transcript release form must be signed by a parent/guardian or student (if over 18 years of age). Each senior will receive a free copy upon graduation. The first year after graduation transcripts will cost the student $3.00. Thereafter each transcript will have a $5.00 fee. Payment must be in the form of cash, money order or certified bank check. No personal checks will be accepted.

Official transcripts will not be sent to students, parents or former students, but only to institutions or organizations designated. Requests must be made in writing and submitted to Maple Heights High School, Attn: Records, One Mustang Way, Maple Heights, Ohio 44137. Please direct questions to 216.438.6400, ext. 1026.


Graduation verification requests must include the former student’s name, date of birth, and estimated date of graduation; and be emailed to or can be sent via regular U.S. mail to Maple Heights High School, Attn: Graduation Verifications/Records, One Mustang Way, Maple Heights, Ohio 44137.
