Cleveland Clinic School Based Health Mobile Unit
Maple Heights City School District partners with Cleveland Clinic to offer the Cleveland Clinic Mobile Unit to our students/families. The Cleveland Clinic Children's Mobile Health Unit offers COVID vaccinations for students as a priority but will include access to the vaccinations for school staff, adults, and the community.
Parents must fill out the Cleveland Clinic School Based Health Care Form in advance of the date. You can fill out the form using one of the Enroll Online links on this page or by scanning the QR code. If your child does not have insurance, a Cleveland Clinic financial counselor will contact you to explore possible assistance options within one week of receipt of the school-based health care form.
As long as an enrollment form is completed, the parent/guardian does not need to be present for their child to be seen by Clinic staff. For more information, call 216-442-7242
